Missing Div Elements in Topic Moderation Template

The topic moderation template (`default.php`) has missing `<div>` elements that cause HTML structure problems compared to the previous template version.
Kunena 6.4.7
Working: KPraxis 6.1.4
Problems: KPraxis 6.1.5
